CO-FAB Workshop

Where imagination sparks innovation!
CO-FAB, which stands for collaboration and fabrication, is a five-week workshop hosted by Form5. CO-FAB provides an opportunity for those with limb differences to work with designers, engineers, and medical professionals to turn their imagination into innovation! Form5's newly developed STEM curriculum for CO-FAB provides ALL participants with an experiential learning opportunity. Over five weekends (Saturdays), recipients co-create a custom-application device with their project teams.

Form5 is proud to announce that two CO-FAB Workshops are planned for the 2023-24 academic year. The Autumn Workshop will take place over 5 Weekends (from 9/16 - 10/14). Applications for the Fall Workshop will open on May 26th, 2023.

How It Works:

Form5 selects 3 individuals with limb differences per workshop to 'co-fab' their own custom-application devices.

The application process begins with meeting the recipient where they are today and understanding their specific needs to help them embrace their future.

NEED A DEVICE?

STUDENTS

Are you a college student majoring in Biomedical, Mechanical, Industrial engineering, and or Design? Are you a college student majoring in a medical-oriented field (physical therapyoccupational therapyathletic training)?

Are you interested in collaborating with industry mentors and other students with diverse skill sets? If you answered yes to all the questions above, you should consider applying to be a part of the CO-FAB cohort! Form5 will select 15 students per CO-FAB Workshop.

INDUSTRY MENTORS

Are you a professional working in an Engineering, Design, or Medical-oriented field?

Are you looking to make a difference with your time and talent? Are you interested in mentoring the future of your profession? If you answered yes to all of the questions above, then you should consider applying to be a mentor for the CO-FAB cohort! Form5 will select 10-12 industry professionals per CO-FAB Workshop.
